Information Day - preparing your young person for adulthood

SNAP Parent Carer Forum are offering this FREE information day, suitable for parents/carers whose young person is between 14-25 years of age. The Advocacy & Support Partnership have been invited to to provide this invaluable training.


Themes that will be explored include:

  • What does the law say for young people with SEN and disabilities, who are approaching 18, or for those who have already reached adulthood?

  • What rights do parents and carers have in providing, or not having to provide, support?

  • What care and support are young people aged 18 or over who have additional needs entitled to under the CARE ACT 2014?

  • What does this mean for young people planning for adult life, and for carers? Or for those already over 18?

  • Who should be assessed, and what should that assessment look like?

  • What should a good care plan look like?

  • How does the care plan link with Education Health and Care Plans (EHCPs)?

  • What are your rights as carers?

  • What role does health play in the assessment of care needs?
